I first posted on this when I was only 100 pages into the book. I've finally finished it by reading it on the bus each morning to work, which made it a slow process. I still think it was as good as I had ranted about it then. I've ended up learning more about cancer by reading this book than I had in all my university studies. If I ever teach at a university, I'm going to make this book the main text for my course. It explains not only everything about cancer, but most major medical history of the last 200 years.
In order to do a proper review of it, I'm going to need several more posts, which I'll be writing up hopefully in the next couple days, but may take more depending on how much free time I get this week. There's so much there, I'm struggling with organization: chronological, topical, facts vs commentary, etc.
One last note for now: this book is now available in paperback, which you can order here.
